Registering for Outpatient Procedures
The Outpatient Registration area is located at the ER entrance to the hospital. Hours of operation are: 7am to 6pm Monday through Friday. If you have questions please send a message to Outpatient Registration or call us at (817) 408-3151.
Pre-Register Whenever Possible
To reduce your wait time, it would be helpful for you to pre-register for procedures 48 hours in advance. All information necessary will be gathered at this time, please bring the items listed above with you. physician order, your insurance card(s), driver's license and social security card with you. All information will be handled confidentially.
Many medical procedures require pre-authorization and financial arrangements. Pre-registration allows time to complete this. It also allows the hospital to verify insurance benefits and to communicate to you the co-pay or coinsurance amounts that you will be responsible to pay the hospital.
Registering for Inpatient Admission
The ER Registration desk is the hospital's admission area. Please come in the Emergency entrance and proceed to the Registration Desk. This is open 24 hours a day.
It is helpful for you to have your physician call (817) 408-3030 to inform us of your pending arrival. All consents and information for admission will be gathered at this location. Please bring the information listed above with you.
Other helpful information to bring with you includes:

a medical history (previous surgeries or medical conditions, allergies, etc.)

a list of all current medications you take including all over-the-counter vitamins and herbal supplements

advance directives if you have one
Personal prescription medication should be left at home. The hospital pharmacy will provide all medications as directed by your physician.
Your room assignment will be determined by the kind of medical care that you are to receive and room availability. As your condition improves, you may be transferred to another unit in the hospital.
