Case Management

Case Management is one of many services provided by LGMC. Whether you are a patient admitted to the hospital or just visiting the Emergency Department, registered nurse case managers are there to offer assistance with many potential needs. Post-surgical rehabilitation, home health or hospice arrangements, or connecting clients with community resources are a few of the many ways the RN Case Managers can assist you. 

Case Managers communicate with insurance companies to keep them abreast of your progress and to assist in finding in-network agencies to provide care, as needed, after your hospital stay. Case Managers are charged with ensuring that you, the customer, are included in your discharge plans as a patient admitted to the hospital as well.
